Therapy for Erectile Dysfunction
ED can occur for numerous reasons and can be treated in various ways, which allow a man to regain his previous sexual function. A physical exam and detailed history including a review of medications is very helpful for diagnosing most causes of ED. Often, before commencing therapy for ED, a proper cardiac workup is needed to ensure the person has the exercise capacity to engage in sexual activity. Some blood tests may be needed to ensure normal levels of testosterone, as well as to rule out diabetes, and other urological conditions.
Treatment
Depending on the cause and severity of your issue, various treatments may be required. The first and most important is the optimization of your health by stopping smoking if needed, weight loss, improved diabetic and hypertension control, as well as coordination with other providers if medication changes are needed. All risks and benefits for all treatments will be explained in detail. If your erectile dysfunction is caused by stress, anxiety or depression, you might benefit that you, or you and your partner, visit a psychologist or relationship counselor.
Treatments for ED may include oral medications such as Sildenafil (Viagra), Tadalafil (Cialis) or Vardenafil (Levitra). These meds cannot be taken if you are on nitric oxide for heart conditions These increase the blood flow to the penis and allow you to get an erection in response to sexual stimulation. Possible side effects include flushing, nasal congestion, headache, visual changes, backache and stomach discomfort. Other treatment modalities
Other medications are the following:
- Topical Medications: These medications stimulate blood flow
- Creams, Suppositories:
- Self-managed injection: Easy self administration of natural substances directly to the penis to induce blood flow and erection
- Penile Vacuum Device: This device fits over the penis and is pumped to achieve an erection via negative pressure. An elastic ring is administered around the base of the penis to hold the erection firm.
- Penile Implants: Prosthesis implanted in the penis, either as bendable rods or a 3- piece inflatable device that is pumped up to achieve an erection.
- Erectile Dysfunction Shock Wave Therapy: Low-intensity harmless shockwaves are applying to the penis in 5 locations in order to reopen veins and develop new circulation to the penis, thereby leading to stronger erections.
Acoustic Wave Therapy for Erectile dysfunction
Low-intensity extracorporeal shock wave therapy is a safe treatment for men with erectile dysfunction and can work to improve, or even cure, ED in some patients. The European Association of Urology guideline for erectile dysfunction recommends this shock wave treatment in mild organic erectile dysfunction patients or poor responders to phosphodiesterase type-5 (PDE-5) inhibitors. Studies suggest men with vasculogenic ED are among the most ideal candidates for shock wave therapy, but it’s not clear if they are the only ones.
If this modality is a possibility for treatment of your erectile dysfunction, this will be discussed with you thoroughly, reviewing the risks and benefits, costs, and potential side effects.
